AxisThyroid currently sees patients who are residents of New York State. You need to be physically in New York at the time of your appointment.

- Anyone who wants a second opinion on their thyroid results
It’s not a fit for acute emergencies or conditions requiring in-person procedures (thyroid ultrasounds, biopsies). Those need a specialist in person.
Step 1: Online Intake
Everything starts with the AxisThyroid intake form. It takes about 10 minutes and covers:
- Current symptoms and how long you’ve had them
- Your thyroid history and any prior diagnoses
- Existing medications, including any thyroid medication
- Family thyroid history
- Any labs you’ve already had done
This isn’t a generic health form. It’s designed specifically for thyroid patients, so we’re capturing the right context before your first call.

Step 2: Lab Work (If Needed)
If you haven’t had recent labs, we’ll order them before your consultation. This includes our full three-axis thyroid panel: TSH, Free T3, and Free T4 — plus antibody testing and other markers based on your history. If you already have recent labs from another provider, you can upload them or share the values during intake. We’ll evaluate what you have and determine if anything additional is needed.
Step 3: Your Consultation
Your first visit is a 45-minute video call with Bernadette Spencer, FNP-BC, FMACP. She’s not a general practitioner running down a checklist — thyroid is her specialty, and it’s personal: she was diagnosed with Hashimoto’s disease herself.
During the consultation:
- We go through your labs together, including what the numbers mean specifically for you — not just against reference ranges
- You describe what you’re experiencing, when it started, what’s changed
- We discuss your history: previous treatments, what helped, what didn’t
- If a diagnosis is appropriate, we make it
- If medication or adjustments are warranted, we discuss options
You leave with a clear plan. Not a follow-up appointment to discuss what was discussed.
Step 4: Treatment and Prescriptions
When medication is appropriate, we can prescribe electronically to any pharmacy in New York. This includes:
- Levothyroxine (synthetic T4)
- Liothyronine (synthetic T3)
- Desiccated thyroid (like NP Thyroid or Nature-Throid), when appropriate
- Combination T4/T3 protocols for patients who don’t respond adequately to T4 alone
We don’t default to one treatment path for every patient. Protocol depends on your labs, your symptoms, and your history.
Step 5: Follow-Up
Thyroid optimization is a process, not a one-time event. Follow-up appointments are shorter — typically 20–30 minutes — and focus on how you’re responding, whether labs need adjustment, and whether your treatment plan needs to evolve.
