You went to your doctor, got a diagnosis, started levothyroxine. Your TSH came back normal at the three-month recheck. On paper, everything looks fine. In practice, you still feel exhausted before noon, your hair is still falling out, and you’ve gained six pounds since starting medication.

You’re not imagining it. And you’re not alone. A significant portion of patients on standard thyroid medication — particularly levothyroxine alone — remain symptomatic despite a “normal” TSH. The problem isn’t always the medication itself. Often it’s how thyroid function is being measured, and what the dosing decisions are actually based on.

Here are five signs your thyroid medication may not be optimized — and what to do about each one.

1. You’re Still Exhausted

Fatigue is the most common complaint among thyroid patients who are supposedly “treated.” If you’re sleeping 8–9 hours and still dragging yourself through the afternoon, or if you need to rest after light activity, your thyroid optimization isn’t complete.

The most common reason: your free T3 is low, even if your TSH is normal.

TSH (thyroid stimulating hormone) is produced by your pituitary, not your thyroid. It tells your thyroid to make more hormone. When you’re on levothyroxine (T4), your pituitary may quiet down and produce less TSH — which makes your TSH look normal on a lab report. But TSH is a downstream signal, not a direct measure of how much active thyroid hormone is circulating in your cells.

T3 is the active form. Your body converts T4 (the medication) to T3. If that conversion is inefficient — due to nutrient deficiencies, high stress, chronic illness, or genetics — you can have normal TSH, adequate T4, and still have functionally low T3. The result is persistent fatigue that no amount of levothyroxine dose adjustment will fix, because the problem isn’t T4 levels.

If your clinician is dosing your medication based on TSH alone without checking Free T3, they are flying with one instrument. You need the full panel.

2. Brain Fog That Doesn’t Clear

Cognitive symptoms are among the most underappreciated in thyroid disease. Difficulty concentrating, trouble finding words, feeling mentally slow, forgetting things you just heard — these are common thyroid symptoms that often persist after starting medication.

The brain is highly sensitive to thyroid hormone levels. T3 in particular plays a critical role in neurological function, mood regulation, and cognitive speed. Some patients don’t convert T4 to T3 efficiently in brain tissue, even when serum T3 looks acceptable. This is one reason why a subset of patients feel cognitively sharper on combination T4/T3 therapy versus T4 alone.

3. Unexplained Weight Gain Despite Normal TSH

Your TSH is in range. You haven’t changed your diet. You’re gaining weight anyway.

Weight dysregulation in thyroid patients is multifactorial, which is exactly why single-marker thyroid testing is inadequate. 4. Hair Loss Continues After Starting Medication

Hair loss is one of the most emotionally distressing symptoms of hypothyroidism. Many patients start medication expecting the shedding to stop. When it doesn’t, or when it slows briefly and then resumes, they’re told their thyroid is fine and the hair loss must be something else.

Sometimes it is something else. But often, it’s still the thyroid — just not being measured correctly.

Hair follicles are extremely sensitive to thyroid hormone levels. Even mild T3 deficiency can disrupt the hair growth cycle. This happens because TSH-based dosing is a proxy for thyroid optimization, not a direct measure of it. The goal of thyroid treatment is not a number on a lab report. It is a patient who functions well — energy, cognition, weight, mood, sleep. When those metrics are ignored and only TSH is tracked, the treatment process becomes self-referential: dosing to normalize a number that doesn’t fully reflect how the patient feels.
